Great Plains’ E-Contracting system allows us to submit your licensing paperwork to carriers electronically, making the contracting process quick and painless!*

Please return the paperwork COMPLETED IN FULL to info@gpam.biz along with the names of the carriers where you wish to be appointed. In addition to the e-contracting paperwork, please also include the following items:

  • Copy of a voided check or letter from bank stating the account holder’s name, routing number and account number. (NO STARTER CHECKS!)
  • Copy of current E&O
  • AML Training certificate
    • If AML was taken through LIMRA, please provide a screen-shot showing your name, AML course and completion date if possible. Since LIMRA does not provide an actual certificate, we can submit a screen-shot to help speed up the process of verifying AML.
  • Copy of your 4-hour Annuity CE Certificate if applicable
  • Current copy of your state insurance license(s)
